Aimes Consulting, Inc. is providing this notice regarding the availability of IRS Form 1095-C, Employer-Provided Health Insurance Offer and Coverage. Form 1095-C contains information about the health coverage offered to you, if any, during the applicable calendar year in which you were employed by Aimes Consulting, Inc.

You may request a copy of your Form 1095-C at no charge.
